PC with a MSI Rx470 4GB GPU, running Windows 10 Pro and AMD Drivers ver. 18.4.1

10m+5m HDMI

Samsung ue55MU6192 TV (4k smart tv, but the GPU sends it only FullHD image)

PC is also connected to a BENQ 24'' display by DVI cable.
I've has this setup for about three months and all has been well up until about a week ago when it started acting up. After some time of turning all of it on (usually around 5-15 minutes of watching something) the screen goes very grainy - like the "film grain" effect. After a minute or two of this, it starts going black for a few seconds at a time, acting like the cable was being yanked in and out repeatedly. At this point I usually turn off or disconnect the TV from the PC.

after I disconnect the TV, the PC seems fine except that the player I've been using (KMP/VLC/MPC) displays no image and is not responding.
The above leads me to believe it's either a driver issue - but I've been updating my GPU drivers every time it tells me there's a new version - or a fault in the GPU itself, which I'm leanign towards since in the past it has mysteriously crashed a few games with a "display device not found" message or similar.
Does this seem about right? Is there something I can do to pinpoint this issue? Should I try a HDMi repeater? (the problem started only recently, also I don't have one and don't want to waste $20 if it's not relevant)

I don't really feel like rolling back the drivers since they did seem to fix the video acting up in some games

nor do I feel like going into the shop to claim warranty just yet, because saying that the problem arises only under specific circumstances seems like a surefire way to get slapped with a "unable to reproduce" response after being without a GPU for a month.

everything works fine when using the PC only with the DVI display, or even when connecting it into the HDMI slot with a DVI->HDMI plug

I have no other HDMI device to test beside the TV
Here's what's going on when I turn on TV and switch the PC to duplicate display, it's pretty much the same as what happens when letting it run for a while - set the video to 1080p to actually see the grain effect:
