Motherboard: ASUSTek H81M-D motherboard

RAM: 8GB

PSU: Corsair CX600-M

Operating System & Version: Windows 10 OS version 1709, upgraded from window 8.1

GPU Drivers: 391.35, clean install

Description of Problem: Last few days I've been experiencing problems with my graphics where whenever I load up a game or certain screens there are these static-like squares that appear (seen here). One time, while trying to fix the issue my PC blue screened. I opened up the PC to check if there was something wrong with the physical graphics card and when I moved it, I heard something loose within it. Think that may be the issue but not 100% sure.

Troubleshooting: I have tried running malwarebytes in case it was some odd virus, tried restarting my PC multiple times, tried updating graphics card driver some times, thought it may have been too much dust in the GPU fan so I cleaned it with compressed air, tried restarting my PC on safe mode, tried removing the graphics driver and resinstalling newest as well as older versions.
